substitute without Noura’s knowledge, as it was believed that Noura
was so addicted that it was difficult to know whether she really
had pain or not. Gradually, Noura’s analgesia was replaced with the
water, despite her complaints of cramps, nausea, and pain. After a
few weeks she was told that her drugs had been substituted and that
it was now possible to control her pain with a milder and safer
analgesic. She was furious that we had deceived her and accused us
of causing her unnecessary suffering. Her sister felt that the
