An 18-year-old student was admitted to the clinic with
complaints of difficulty in exhaling, a painful cough with a small amount of viscous sputum. These
symptoms appeared 3 days ago after physical education. In childhood, he suffered from
allergic rhinitis, episodes of dry cough were noted. On physical examination: the patient is cyanotic, the
respiratory rate is 22 per minute, the pulse is 100 beats per minute. Resistance of the chest on palpation is
noted, voice tremor is weakened on both sides; with comparative percussion – a symmetrical box
sound. Auscultation of the lungs – vesicular breathing is weakened, the expiratory phase is
lengthened, multiple polyphonic wheezing rales on both sides. 1. Name and justify the syndromes. 2. Explain the mechanisms of the formation of polyphonic
wheezing in this patient. 3. What laboratory and instrumental studies should be assigned
and what are their possible changes? 4. Formulate the clinical diagnosis according to the
classification.
